Service

Proactive visibility.
Consistent connection.

Regular wellness check-ins — purposeful, structured touchpoints by phone or video — help catch changes before they escalate. Available to families anywhere in the United States. For distant families, they provide reliable reassurance. For clients, they provide consistent connection and a trusted presence.

The changes that matter
most happen gradually.

A decline in appetite. Increasing confusion in the evenings. A subtle shift in mobility. Withdrawal from activities a person used to enjoy. These changes rarely announce themselves dramatically — they accumulate quietly, week by week, until something finally breaks.

Regular wellness check-ins create the consistent baseline that makes early recognition possible. When someone sees your loved one regularly and knows what "normal" looks like for them, they notice when something has shifted — often long before a crisis.

For families managing care from a distance, check-ins provide the reliable, eyes-on presence that replaces worry with information.

For distant families
Reliable reassurance. You'll know — with confidence — how your loved one is actually doing, not just how they say they're doing.
For clients
Consistent connection with a trusted face. A regular check-in with someone who genuinely cares is itself a form of support.
For the care team
Ongoing documentation of how a client is functioning between clinical appointments — giving providers a fuller picture when it matters.

Structured, purposeful,
documented.

01
Virtual Wellness Visits
Structured video calls — conducted via FaceTime, Zoom, or phone — organized around wellness observation, conversation, and a clear checklist of what we're looking for. When helpful, we guide the client or a trusted person to show us the home environment on camera. Available to clients anywhere in the US.
02
Virtual Check-Ins
For clients who prefer phone contact, or as a lighter-touch supplement between video visits — video or phone check-ins that maintain consistent connection and provide meaningful wellness observation without requiring travel.
03
Written Family Reports
After each check-in, a clear written summary is shared with family members — covering what was observed, any changes noted, and recommended next steps. Consistent documentation builds a picture over time that's invaluable for the care team.

A comprehensive look at
the whole person.

Each wellness check-in is structured around a consistent framework — so changes over time are tracked meaningfully, not anecdotally.
