Consistent support for
conditions that don't take
a day off.

Managing a chronic illness is its own full-time job — for the person living with it, and for the family trying to help. Avelis provides structured, ongoing support that keeps clients stable, informed, and proactive between medical appointments.

The space between
appointments is where
most things go wrong.

A 15-minute appointment every three months was never designed to manage a condition that affects your loved one every single day. Medications change. Symptoms shift. New concerns emerge. And in that gap between visits, families are often left without guidance, without a trusted contact, and without a plan.

Avelis fills that gap — with ongoing oversight, proactive communication, and the kind of structured support that keeps small changes from becoming serious crises.

Our Role
We don't replace the clinical team. We coordinate and reinforce everything around it — so your loved one gets the most from every provider they see.
The Difference
Prevention-focused. Not reactive. We watch for the early signs that something is shifting — before it requires an ER visit.

We work alongside families
managing many chronic conditions.

Our support is not condition-specific — it's person-specific. Below are some of the most common conditions among the families we serve.

Structured support across
every critical dimension.

01
Ongoing Wellness Monitoring
Regular virtual or telephonic check-ins — by video or phone — that track how your loved one is feeling, functioning, and coping. Available to families anywhere in the United States. We document changes over time and communicate proactively when patterns warrant attention.
02
Medication Organization Assistance
Chronic illness often means multiple medications across multiple conditions. We help organize, track, and support adherence — and flag potential discrepancies for review by the clinical team.
03
Care Plan Education & Clarification
We help clients and families understand what their providers have recommended — translating clinical language into clear, actionable information so nothing gets lost in translation.
04
Symptom Tracking & Documentation
Consistent documentation of symptoms, changes, and concerns — so when an appointment comes around, there's a clear picture of what's happened in the weeks prior, not just what the patient remembers that morning.
05
Provider Communication Support
Supporting communication between your loved one, their family, and their medical team — ensuring concerns are raised, follow-up questions are asked, and care transitions are coordinated smoothly.
06
Family Updates & Crisis Prevention
Regular family briefings, early escalation of concerning changes, and proactive communication designed to keep families informed and crises from catching everyone off guard.

What families ask us about
chronic care support.

My parent already has a specialist. What does Avelis add?
Specialists provide excellent clinical care — but typically in short, infrequent appointments. Avelis fills the space between visits: monitoring how your parent is doing day to day, ensuring they're following through on recommendations, and catching early signs that something needs prompt clinical attention.
Does Avelis serve clients outside of Florida?
Yes — all Avelis services are delivered virtually and by phone, so we serve families across the United States. Based in St. Petersburg, Florida, our virtual model means geography is never a barrier. We coordinate care remotely with the same depth and consistency as any local service.
Is this available through a membership plan?
Yes. Our Chronic Care Support Membership is designed for ongoing engagement — available through our Comprehensive or Concierge plans. For families in a shorter-term situation (a new diagnosis, a care transition), standalone engagements are also available.
