Membership

Ongoing support,
not just one-time help.

The families who benefit most from Avelis are those who need consistent, proactive support — not just a visit when something goes wrong. Monthly membership. No long-term contracts. A real person who knows your family's situation.

Consistent support that
evolves with your family.

Every membership begins with a thorough onboarding assessment — a dedicated session where we learn your loved one's health situation, living environment, family structure, and goals. From that foundation, we build a personalized support plan that's specific to your family.

Membership plans are billed monthly with no long-term contracts. We believe families should stay because the support is genuinely valuable — not because they're locked in.

All plans include a dedicated support coordinator, responsive access to our team, and regular family communication.

i.
Onboarding Assessment

A dedicated session to learn your situation — health history, current challenges, family structure, and goals. This is where your personalized support plan begins.

ii.
Dedicated Coordinator

One person who knows your family's situation deeply — not a rotating staff of strangers. When you call, someone who knows your name answers.

iii.
Adaptive Support

As your loved one's needs evolve, your support plan evolves with them. We conduct quarterly plan reviews and adjust proactively — not reactively.

iv.
No Long-Term Contracts

Month-to-month. No lock-in. You stay because the support is genuinely valuable — not because you're contractually obligated to.

Three levels of support.
One dedicated relationship.

Advocacy Accompaniment
An Avelis team member accompanies your loved one to a medical appointment — preparing them beforehand, documenting during, ensuring all concerns are raised, and providing a full debrief with the family afterward.

What families ask about
membership.

Is there a minimum commitment period?
No. Plans are billed month-to-month with no long-term contract. We believe families should stay because the support is valuable — and most do, because it is. You can pause, adjust, or cancel at any time with appropriate notice.
How is my plan customized?
Every membership begins with an onboarding assessment — a dedicated conversation where we learn your loved one's health situation, goals, and family structure. From there, we build a specific support plan within your chosen tier. The tier sets the framework; the plan makes it personal.
Does insurance cover Avelis membership?
Avelis services are typically private-pay and are not covered by Medicare or standard health insurance, as they fall outside the definition of skilled home health care. Some long-term care insurance policies may cover non-medical care coordination or support services. We're happy to help you review your policy during your consultation.
Can I upgrade or change my plan over time?
Yes — and this is common. Many families start with the Essentials plan and move to Comprehensive as needs grow. We conduct quarterly plan reviews and proactively recommend adjustments when your situation changes.
What's the difference between the Comprehensive and Private (Concierge) plans?
The primary differences are frequency and access. The Private plan includes weekly in-home visits, direct coordinator phone access, appointment accompaniment, and priority crisis response support — designed for clients who need more intensive, more frequent engagement. Comprehensive is best for families who need consistent coordination and advocacy without the full weekly in-person presence.
