Service

One person who
holds the full picture.

When care is scattered across multiple providers, care plans, and family members — nothing connects. Avelis serves as your coordination hub, organizing everything into a clear, manageable picture.

Healthcare was not designed
to communicate with itself.

Your parent sees a cardiologist, a primary care physician, an endocrinologist, and a pharmacist — none of whom consistently talk to each other. Referrals get lost. Discharge instructions conflict with what the specialist said last week. Medications are adjusted by one provider without the other knowing.

Families are left to fill that gap — often without a medical background, often from a distance, always without enough support.

That's where Avelis steps in.

"For the first time in two years, I didn't feel like I was managing my mother's healthcare alone. Avelis brought clarity to everything."
— Adult daughter, St. Petersburg, FL

Coordination that covers
every critical connection.

01
Provider Communication Support
We help facilitate communication across your loved one's care team — tracking referrals, following up on recommendations, and ensuring that what one provider says reaches the others who need to know.
02
Medical Records Organization
A centralized, organized picture of your loved one's health history, current medications, diagnoses, and active care plans — prepared and maintained so nothing is ever missing when it matters most.
03
Appointment Preparation & Follow-Up
We prepare clients and families before appointments — relevant history, questions to ask, what to watch for — and follow up afterward to ensure recommendations are understood and actioned.
04
Referral & Specialist Tracking
Referrals fall through the cracks constantly. We track every open referral, confirm appointments are scheduled, and follow up when things stall — so your loved one doesn't wait months for care they needed weeks ago.
05
Family Updates & Communication
Regular written summaries shared with family members — no matter where they live — so everyone is informed, aligned, and never left wondering what's happening with their loved one's care.
06
Medication Organization Assistance
Helping clients organize, understand, and track their medications — reducing confusion, supporting adherence, and flagging potential discrepancies for clinical review by their medical team.

support model.

What families ask us
about care coordination.

Does Avelis replace my loved one's medical team?
No — and that's an important distinction. We coordinate around the medical team, not instead of them. Our role is to help organize, communicate, and ensure that clinical recommendations are followed through. We complement your loved one's providers; we don't replace them.
My parent lives in a different state than me. Can Avelis still help?
Absolutely — and this is one of the most common situations we support. Because all Avelis services are delivered virtually and by phone, we can coordinate care for your parent regardless of where either of you lives. We join appointments by phone, conduct check-ins by video, coordinate with providers remotely, and keep you fully informed with regular written updates.
What does a care coordination engagement actually look like day to day?
It depends on your loved one's situation. For some families, it means a weekly check-in, tracking two upcoming specialist appointments, and a bi-weekly family update email. For others, it means full active coordination across five providers during a complex medical period. We scale to what's needed.
Is care coordination available as a standalone service or only through membership?
Both. Care coordination is included in our Comprehensive and Concierge membership plans, and is also available as a standalone engagement for families navigating a specific period — a hospitalization, a new diagnosis, or a care transition.
